This Form is a Certificate of Formation of a Single Member Limited Liability Company. It is a simple Form, but is generally all that is required (along with a required filing fee) to create an LLC and register it with its State of Formation. Once the Certificate of Formation has been filed, the next step is to prepare an Operating Agreement that sets forth the ongoing governing rules for the LLC.
